Transition to Adulthood
It is vital that families and school teams begin planning for transition well before the date of graduation. Transition goals should be listed in the IEP starting at the age of 14. Teams should also consider how any assistive technology needs will be met after graduation.

Planning for the Future Checklist:
The checklist is designed to support youth with disabilities, as well as their family and IEPteam members, to ensure they are reviewing all aspects of transition planning at age appropriate intervals.
